دخترخانمی با بهم ریختگی شدید دندانها بخصوص در ناحیه چپ بالا مراجعه نمودند. طوری که دندان نیش کاملاً خارج از قوس بود و فضایی برای آن وجود نداشت.
فک بالا جلوتر بود و چاره ای جز کشیدن دندان در فک بالا نبود. در سمت راست فک بالا قبلاً یک دندان آسیاب کوچک کشیده شده بود.
برای اینکه اوایل درمان دستگاه ارتودنسی کوچکتر و بیمار راحت تر باشد و همچنین افزایش سرعت کار، در ابتدا دندان چهار کشیده شد و برای پایین کشیدن دندان نیش و بستن فضا از تکنیک سگمنتال استفاده شد.
بعد از قرار گرفتن دندان نیش در محل صحیح دستگاه ارتودنسی ثابت بطور کامل قرار داده شد.
در فک پایین درمان بدون کشیدن دندان و همراه استریپینگ ( تراش مختصر دندانها ) صورت گرفت.
